I would like to add a few points to this discussion:
1. There is insufficient solid clinical data attesting to the efficacy of SLT in all forms of juvenile glaucoma. However, I think SLT merits a try when necessary, especially in view of the efficacy and availability of other treatment modalities.
2. The only non-transient complications ever reported after SLT were 4 cases of paradoxically increased IOP in heavily pigmented eyes, 3 being severe enough to require trabeculectomy [Harasymowycz, Am J Ophthalmol. 2005]. It is well to take special care in heavily pigmented eyes by lowering the laser "dosage" and monitor the results carefully. However, one may note that SLT results in black and white people are similar.
3. Coagulative necrosis induced by SLT is unlikely due to SLT's energy being 4 orders of magnitude less than that of ALT or laser photocoagulator. Photoacustic damage is also unlikely for the same reason. Since the mechanism whereby SLT affects the trabeculum to reduce IOP is not precisely known, the mechanism of thoce exceedingly rare side effects is purely speculative.

Madhu,
I think coagulative necrosis is impossible with SLT. Perhaps you mean photoacoustic trabecular damage.

Yes, SLT is said to be minimally invasive laser treatment and power setting is meant to be sub threshold but re-treatment of the same point may result into coagulative necrosis as was seen with ALT

1) The treated area was 360 degrees.
2) The power setting was 0.8
3) No. of shots were 103.
4) The angle showed moderate pigmentation.
I stopped the use of topical steroids to the patient and gave topical NSAI, together with topical Alphagan and Cosopt. I was waiting to see the patient again.

I have tendency to re-treat the same point on the TM during the same session, is that harmful?

Madhu Nagar, UK
Reason for raised IOP post SLT are
a) 360 degree treatment in a heavily pigmented TM,
b) Re-treatment of same area as energy delivered to same area will be doubled,
c) High power setting,
d) Steroid response etc
I have minimal experience with Juvenile glaucoma, I have treated 3 patients 5 eyes, three eyes responded well to treatment but 2 eyes were non responders.

Madhu Nagar, UK
My questions are:
1) What was the area treated?
2) What was the power setting?
3) Number of shots given?

I had treated 2 patients with juvenile glaucoma and I recorded abnormal increase in the IOP one week after SLT reaching up to 40 mmHg (pre-SLT 22 with single medication), could it be a steroid response? How to manage this situation?
